Job Description
ProMedica at West Deptford is currently looking for full time Social Services individual for our Skilld Nursing Facility in Paulsboro, NJ. The ideal candidate will have prior Long Term Care and managerial experience.

DUTIES:
- Directs, establishes & plans the overall policies/goals for the Social Services Department
- Responsible for assessments, care planning & helping residents adjust
- Familiar with a variety of the field's concepts, practices & procedures
- Relies on extensive experience & judgment to plan and accomplish goals
- A wide degree of creativity and latitude is expected
- Occasional Community Outreach
- Meet with residents to discuss the financial plan
- Gather documentation for Medicaid applications
- Collect Private funds due to the nursing home
- Assist with insurance authorizations
