Located throughout the New Jersey Counties of Passaic, Morris and Sussex, we provide professionally based social services, including Behavioral Health Programs, Veterans Services, Children’s Programs, Senior and Aging Services and much more.
Early Intervention - Service Coordinator
Hours:
8:30 am to 4:30 pm Monday through Friday. Some flexibility in schedule is required in order to accommodate family schedules.

Salary: $43,000.-$45,000.
Job Summary:
The Service Coordinator is responsible developing service plans that identify and meet the needs of eligible children and their families. Ensuring that appropriate services are in place according to the service plan and monitoring these plans for timely and continuous services. Responsible for assisting and enabling child eligible under NJEIS and the child’s family to receive rights, procedural safeguards, and services authorized under the NJ Early Intervention System.
1) Conducting a Family Information Meeting (FIM/FDA) according to the procedures outlined by the NJEIS.
2) Facilitating and participating in the development of an Individual Family Service Plan (IFSP) within the 45 Day Timeline requirements as mandated by NJEIS.
3) Completing all FIM and IFSP paperwork to be submitted to the SPOE data department in the region in accordance with NJEIS policies and procedures.
4) Adding service needs to the twice weekly broadcast in order to assure timely provision of service to client, and ensuring 30 Day Timely Service requirements as mandated by NJEIS.
5) Informing families of availability of advocacy services and referring as appropriate.
6) Communicating with medical and health providers and assisting with the coordination of services as appropriate.
7) Facilitating the development of a transition plan to preschool or other community services, by conducting Transition Planning Conferences with clients, agencies and school districts 90 days, or greater, prior to the client’s 3rd birthday as mandated by NJEIS.
8) Maintaining an accurate and organized client record, including documentation of all case work activities, most importantly relating to mandated NJEIS timelines.
9) Maintaining professional relationships with peers in outside agencies.
10) Working cooperatively as a member of a team as well as independently.
11) Representing Catholic Family and Community Services and Special Child Health Services in the community.
12) Participating in community activities and on committees to build community relationships.
13) Attending all training sessions required by the NJEIS and CFCS.
14) Flexibility in scheduling appointments with clients.
15) Maintaining current professional license / certificate necessary for the position.
16) Responsible for other duties that may be assigned from time to time to support
effective and efficient operation of SCHS and Catholic charities.

Skill and Requirements:
1) Must have RN, CSW, LSW, LCSW, or Bachelors degree or higher from an accredited institution in a related health, human service, or education field with one full year of documented professional experience working with children and families in a counseling, supporting or case management role. Preferably with individuals birth to five years of age and their families.
2) New Jersey professional license/certification must be current.
3) Must have a valid driver’s license and car.
4) Preferable to have knowledge and understanding of the following:
a) Early childhood development
b) Part C of the Federal Guidelines and NJ Rules and Regulations
c) Nature and scope of services available under NJEIS (https://www.state.nj.us/health/fhs/eis/documents/njeis_pra.pdf)
system of payments and other pertinent information.
d) Must be able to access community and state resources in order to assist families.
5) Bilingual Spanish and/or Arabic a plus.
Position requires working in an office as well as visiting homes of clients and travel to meetings throughout the county and state
