Organization: A Non-profit Organization serving the blind and visually impaired, Lions Center for the Visually Impaired preserves vision, fosters independence and enhances the quality of life for adults in the East Bay who are blind or at risk for vision loss and serves as a trusted source of information and expertise on blindness in the community. Our services focus on early detection and intervention in order to prevent or slow vision loss from age-related eye disease. We also provide older adults in the East Bay with the tools and skills they need to maintain their health, independence and quality of life despite vision loss.
Job Summary: The Vision Screening Coordinator is responsible for the implementation and coordination of Vision Health Screenings, and for the follow up and reporting of client services. Requires a well organized, flexible team player. Must travel throughout Contra Costa, Alameda, and Solano counties. S/he will be able to balance the programmatic demands, manage time effectively, and work individually and as a team-player. Attend weekly staff meeting.
Qualifications: High School Diploma/GED required. 2-3 years experience working with senior client services and/or outreach. Previous work in vision services a plus. Well-organized; ability to multi-task, good record keeping skills and the ability to get along with a variety of individuals. Passion and caring for supporting the needs of blind and visually impaired population with the goal of providing the highest quality of care and independence.
Accountabilities:
· Establish and coordinate eye screenings targeting senior living facilities in Alameda, Contra Costa and Solano counties.
· Initiate outreach to the target population by telephone, email, mail and site visits in the community.
· Ability to effectively utilize Eye Screening mobile machines.
· Initiate and maintain relationships with Social Coordinator, Social Workers and Administrators of senior living facilities, creating service opportunities for eye screenings and outreach for other services within the organization.
· Schedule screenings and coordinate logistics with facility’s Social Coordinator, LCVI staff and volunteers.
· Attendance at eye screenings and responsible for all aspects of screening including staff, volunteers, Social Coordinators and residents.
· Manage follow-up with residents/clients referred or recommended for treatment which may include working with medical industry, insurers, transportation providers, Lion Eye Foundation and other providers as needed for a positive client outcome.
· Manage the eye glass program which includes responsibility of sending the certification to clients and recording data regarding the receipt of the certification.
· Maintain confidential records and compiles data on all clients.
· Report findings and outcomes to grant providers and executive staff.
· Support marketing and outreach of services to new providers.
Compensation: The Lions Center for the Visually Impaired will offer a competitive salary of $18-23 per hour, based on experience. Full-time, 40 hours per week. Benefits include medical coverage, paid holidays, vacation, and sick time.
