AbilityPath is hiring for a Family Resource Coordinator!
The FRC Coordinator works with families of children with disabilities and service providers to support referrals, information, education, and parent to parent support.
Candidates must be a parent or family member of a child or adult with a developmental disability or have special needs.
We are hiring for 2 full-time coordinators at 30 hours/week + benefits!
Bilingual in English/Spanish or other high-frequency languages in San Mateo County such as Portuguese, Mandarin, Cantonese or other Asian/Pacific Islander languages required

ABOUT THE ROLE:
- Answer warmline calls, emails, and web inquiries and provide resource and referral information to parents/caregivers
- Assist families at the time of transition from Early Intervention services to School District Services as well as support families with transition to adult services as applicable
- Track all case notes and outreach efforts in ALICE database in a timely manner
- Coordinate and perform outreach activities to families who have children with disabilities both virtually and in person
- Coordinate and perform outreach activities to community providers to promote referrals to the Family Resource Center and track in ALICE
- Collaborate with Help Me Grow San Mateo County (HMG SMC) team and accept warm referrals from HMG SMC team
- Continue and expand parent support activities
- Actively engage and participate in FRC training events and support groups
- Actively engage and participate in team meetings including Case Review Meetings, Staff Meetings and Reflective Practice Meetings
- Other duties as assigned
EDUCATION AND WORK EXPERIENCE:

- Education preferred in child-development, social work, psychology, or related field
- Relevant work experience with families and children birth through age 22
- Bilingual in English/Spanish or other high-frequency languages in San Mateo County such as Portuguese, Mandarin, Cantonese, or other Asian/Pacific Islander languages.
HIRING REQUIREMENTS:
- Valid California Driver’s License/ID
- Clean Driving Record (if applicable)
- Proof of negative TB test results
- Verification of occupational physical clearance
- Pass a criminal fingerprint clearance
- 18 years of age or job permit clearance
- Proof of Covid-19 Vaccination + first booster
PHYSICAL ACTIVITY:
Ability to sit/stand and keyboard- Ability to lift at least 25 lbs.
- Ability to drive and travel locally
