Family Service Coordinator
SUMMARY:
The Family Service Coordinator is responsible for overseeing all Innovations Waiver services in conjunction with the MCO and other case management organizations. This includes, but is not necessarily limited to, managing direct care staff and coordinating service delivery to ensure that client needs, program goals, and contract outcomes are effectively met.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned.
- Establish solid working relationships and maintain on going communication with all agencies involved in case management and placements
- Establish and maintain solid working relationships with the MCO’s, guardians, and other key stakeholders in the community
- Develop and follow guidelines and criteria for assisting the process of direct care staff, management team, and the agency in meeting the above stated goals.
- Remain current on changes with the Innovation waiver and suggest appropriate program changes as necessary.
- Ensure compliance with applicable federal, state, and agency statutes, administrative codes, policies, and procedures.
- Oversee case assignment and services delivery
- Provide support with dealing with crisis management issues.
- Coordinate efforts with the Director of Operations to ensure the provisions of regular training to new and existing staff and to ensure that minimum training requirements are met;
- In conjunction with the MCO’s participate in Quality Assurance activities, including ensuring that case files are reviewed and monitored on a regular basis for both internal and external quality assurance standards;
- Ensure that services delivery complies with contractual requirements
- Review assessments and case plans with treatment team and provide consultation/direction to assure appropriateness, clarity, quality and thoroughness;
- Provide guidance to direct care staff completion of all documents in a timely and professional manner;
- Prepare and conduct staff performance evaluations, including following procedures for progressive discipline when appropriate;
- Generate and maintain necessary reports and paperwork I a timely manner;
- Participate in meetings with other administrators and outside agencies;
- Collect, analyze, and report data in area of responsibility;
- Treat all clients, coworkers, and colleagues with courtesy, dignity, and respect;
- Adhere to safety standards in the workplace;
- Advise agency of significant events and pertinent issues;
- Assure compliance with MCO contracts;
- Organize and facilitate day program activities

PREFERRED SKILLS:
Education and Experience:
- Master’s Degree in social work or a comparable human services field from an Accredited institution and a minimum of five years of I/DD experience with at least two years at the supervisory level; or
- Bachelor’s Degree in a human services field from an accredited institution and a minimum of ten years of I/DD experience with at least five years at the supervisory level;
- Demonstrated knowledge and understanding of the Innovation waiver system;
- Demonstrated knowledge and understanding of the accreditation system with CARF.
Language Skills:
- Ability to read, analyze and interpret regulatory documents, contracts, and policies.
- Ability to respond to common inquires or complaints from management and staff.
- Ability to effectively present information to management and employees.
Reasoning Ability:
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Ability to deal with several abstract and concrete variables simultaneously.
- Ability to anticipate needs and/or problems based on current events and/or situations and to develop solutions.
Training Requirements:
- Complete at a minimum of 20 hours of in-services training each full year of employment.
- The in-services training must include, but is not limited to, EBPI, First Aid & CPR, CORE, Medication Administration, Seizure Management, Health/Safety and Blood Borne.
