The Community Health Worker is a non-medical staff member. The purpose of this position is to build trust and a relationship and engage the member facing difficult issues. The position serves as the primary connection as well as a bridge for the highest risk members to support a broad array of health care and social issues. In this position, the CACS advocates both within the community and within the healthcare system.


This role is equally challenging and rewarding. You'll need to stay positive at all times especially in difficult situations. You'll be asked to mentor, challenge and communicate effectively with all different types of people.


This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00am – 5:00pm. It may be necessary, given the business need, to work occasional overtime and/or weekends.



Primary Responsibilities:



  • Work with members to develop healthcare goals and identify potential barriers to achieving these goals

  • Identify member support systems available and incorporate into plan of care

  • Traveling to local shelters and community organization and members’ homes to assist members with healthcare needs

  • Collaborate with member to create solutions to overcome barriers to achieving healthcare goals

  • Identify relevant community resources available based on member needs

  • Refer members to appropriate programs and services

  • Present complex case information to interdisciplinary care team and obtain input into member plans as necessary

  • Coordinate with providers to facilitate member care and/or services

  • Provide guidance or additional assistance to facilitate execution of plans or recommendations to improve member health

  • Assist with transportation needs

Required Qualifications:


  • High School Diploma / GED (or higher)

  • Have resided within the local community for 2+ years

  • Experience with the resources available within the community

  • Microsoft Office including Microsoft Word (create correspondence and work within templates), Microsoft Excel (ability to sort, filter, and create simple spreadsheets), and Microsoft Outlook (email and calendar management)

  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

  • Ability to travel 75% of the time in local areas

  • Ability to work an 8-hour shift Monday-Friday from 8:00am to 5:00pm

  • Reside in Las Vegas, NV area

  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:


  • Bilingual fluency in English and Spanish

  • Experience of Medicaid and / or Medicare health care and insurance industry

  • Experience of culture and values in the local community

  • Field Based experience

  • Community Health Worker (CHW) Accreditation

  • Experience working in Managed Care

  • Possess strong organizational and time management skills

  • Possess the ability to multi-task and prioritize tasks to meet all deadlines

  • Possess ability to work well under pressure in a fast-paced environment

  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

The salary range for Nevada residents is $20.00 - $35.53. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
