Forensic PATH Case Manager Job at Community House Mental Health Agency
Community House is looking to fill the position of Outreach and Engagement Case Manager- Behavioral Health
Employment Type: Full-Time (35 hours/week)
Schedule: Mon-Fri 9:00AM – 4:00 PM
Salary: $50,000/DOE plus ability to earn quarterly incentives

Program Summary (may fluctuate):
The FPATH Program works with adults with behavioral health conditions who have had multiple incarcerations where competency was raised. Referrals to the program come through the Washington State Health Care Authority based on data submitted courts. The program works to outreach and engage clients on the state list with the goals of providing short term, intensive, companion-based case management with the goal of connecting individuals to long term mental health and/or substance use treatment and social services resources. Individuals do not have to be Medicaid enrolled to be eligible for FPATH and can be served by other agencies at the same time as FPATH participation. The team provides individualized outreach case management to connect participants to resources including housing and/or shelters, DSHS and SSI benefits, food, transportation, legal support, medical, and any other needed resources.
Outreach and Engagement Case Manager Job Summary (may fluctuate):
This position works as part of a team to provide FPATH services, to meet established FPATH Program goals, by bringing services directly to the client wherever they may be. This may include their home; shelters; streets; family; other community services and programs. Outreach and Engagement Case Managers will work within a team model to provide needed services, linkage, and coordination. Coordinating with other providers is an integral part of the job. The FPATH team utilizes a harm-reduction and trauma-informed approach.
Minimum Qualifications:
- Bachelor’s Degree in social services field with experience working with individuals with severe and persistent mental illness, individuals with chronic substance use issues, and individuals involved in the criminal justice system is preferred.
- Demonstrated knowledge and experience working with community mental health, individuals with severe and persistent mental illness who come into contact with local criminal justice system is essential.
- A working knowledge of co-occurring behavioral health issues is preferred.
- A working understanding of systemic inequities the population experiences from a social justice framework.
- Sensitivity to multi-cultural populations and issues
- Valid Washington state Driver’s License
Essential Job duties:
- Outreaching clients where they are living/staying and in the broader community
- Develop rapport with individuals with extreme barriers to engaging in services
- Driving in an urban setting to outreach clients and assist in transporting clients to appointments in agency vehicle. Assisting clients with utilizing public transportation or other forms of transportation.
- Coordinating with other professionals (behavioral health, legal , medical, social service, other)
- Completing needs assessment and goal setting for development of Recovery Plan
- Support clients with following up with legal expectations with goal of reducing recidivism
- Engaging with clients who are under the influence of drugs and or alcohol and clients in severe poverty who may have poor hygiene
- Harm Reduction approach with demonstrated passion for serving individuals with co-occurring mental health and substance use disorders who are at various stages of change
- De-escalation skills/strategies and crisis intervention and risk assessments
- Work effectively as part of a multidisciplinary team
- Advocacy for clients and assisting clients in learning to advocate for themselves
- Timely completion of required documentation to program standards
- Adhere to HIPPA/Confidentiality Policies

- Employee will be required to sit, walk, reach, bend, grasp, stand, twist, squat, kneel, push, pull and lift and carry items weighing 25 pounds.

Skills Required:
- Good Diagnostic and assessment skills along with expertise with Co-Occurring disorders is a necessary part of the position.
- Knowledge of or background in working in the forensic/competency system; individuals who do not have some aspect of this experience will be considered, and training will be provided upon hire.
- An understanding of case management approaches to provide services to individuals with psychiatric disability.
- Good working knowledge of community and social service resources in King County and Washington State.
- Familiarity with state and local mental health delivery systems, including knowledge of various funding principles and clinical service mandates.
- Ability to organize ones’ own work, to work independently and to initiate collegial, supervisory, and consultative work.
- Capacity to appreciate and work well with individuals who have high mental health needs and maybe experiencing homelessness and living in extreme poverty.
- Good diagnostic assessment skills.
- The ability and skills to work as a team member and understand the criminal justice system.
- Cultural competency and a working understanding of providing culturally responsive and culturally informed services, including a commitment to the City of Seattle Race and Social Justice Initiative and the King County Equity and Social Justice Initiative.
- Ability to navigate and use technology (laptops, cell phones, virtual meetings with other providers/systems, Microsoft office)
- Flexibility with the workday and ability to multitask while still meeting the requirements of the program.
