Position Summary:
The Registered Nurse (RN) Provides direct nursing care to the residents in a safe manner and supervises the day-to-day nursing activities performed by the certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. Providing and teaching medical diagnosis, medications uses and side effects, and performing nursing procedures, assessing, and reporting resident change to provider.

Major Responsibilities:
The Registered Nurse will:
  • Provides care to residents as planned by the interdisciplinary Care Plan Team and uses safe techniques which are in accordance with the Upjohn Community Care Center and regulatory policies and procedures.
  • Assists the resident to achieve and maintain their highest level of functioning in accordance with the care plan and provide input to the Nursing Supervisor for appropriate care plan changes.
  • Consistently works cooperatively with the Interdisciplinary Care Plan Team and families to develop, implement, and review plans of care. Maintains the resident record utilizing the electronic medical record.
  • Must use acceptable infection control procedures including Universal Precautions when providing care to residents.
  • Cares for medical equipment and supplies used for residents to ensure proper care and charges are made
  • Accurately completes documentation in the medical record to reflect care rendered and reports significant changes in resident condition to the provider upon completion of the change of condition assessment or focused assessment using the call guidelines provided by Borgess Extendicare.
  • Attends training programs and in-services to meet Heritage Community of Kalamazoo, the Upjohn and regulatory guidelines, including but not limited to annual skills fair and skills check.
  • Maintains a positive attitude and contributes to the facility interdepartmental team by completing assignments in a timely, accurate and professional manner.
  • Complies with HCK Customer Service Standards and the Upjohn’s- CNA Standards of Care.
  • Follow Resident Rights at all times.
  • Assist new employees in following established Heritage Community of Kalamazoo and regulatory policies and procedures.
  • Reports inoperable, faulty, or unsafe conditions, equipment, and furniture to Nursing Supervisor.
  • Must be able, during a facility emergency/disaster, to report to an assigned area, which will be assigned by their supervisor, and perform the necessary tasks to ensure the calm, orderly and safe handling of residents, staff and public to either move them to safety and /or remove from the threat causing the emergency/disaster.
  • Other duties as needed or assigned by supervisor.
Required Licenses, Registration, Certifications:
  • Must be a Registered Nurse (RN) in the state of Michigan
  • Unencumbered by any licensing or state violations
  • Must also maintain the required CEU’s
Knowledge and Critical Skills/Expertise:
  • Must be able to read, write and comprehend English.
  • Must be able to follow written and oral instructions.
  • Must be able to safely lift, transfer and ambulate residents.
  • Must possess psychosocial skills to be able to interact effectively with residents, staff, families and visitors.
Essential Physical Functions:
  • Able lift 50+ pounds
  • REACH above shoulder height: Frequently occurs while reaching for supplies on upper shelves.
  • REACH at shoulder height: Constantly occurs while documenting in chart, setting up medications, and providing care to residents.
  • REACH below shoulder height: Frequently occurs when reaching for clerical supplies or charting information in drawers or medications in lower parts of med cart.
  • PUSH/PULL: Frequently required to push/pull medication cart, desk drawers; or resident wheelchair.
  • Grasping: Constantly during course of day while documenting medical files. Frequently while transferring residents. Frequently while handling specific nursing equipment.
  • Handling: Frequently handling paperwork and occasionally various diagnostic tools such as thermometer, blood pressure gauge, resident personal items, etc.
  • Torqueing: Required for opening some medications. Required for removing tops from tubes, tops of bottles, twisting linens.
  • Fingering: Required for using typewriter, computer keyboard and telephone. Frequently while documenting residents’ medical charts. Completing physical assessment of resident.
  • Twisting: Occasionally occurs while transferring resident.
  • Bending: Frequently occurs when drawing blood, taking vital signs or setting up and dispensing medication.
  • Crawling: Occasionally occurs when items are dropped to floor and roll under the objects.
  • Squatting: Occasionally occurs when making contact with resident in wheelchair or chair.
  • Kneeling: Occasionally occurs while examining lower extremities of resident.
  • Crouching: Frequently required when doing foot care or making eye contact with resident in wheelchair or seated.
  • Climbing: Occasionally occurs during CPR or emergency procedures.
  • Balancing: Frequently required while ambulating residents in rooms or hallways, or carrying items from one area to another; and when performing CPR.
  • Standing and walking varies form carpeting, linoleum, and tile. Hard surfaces often wet and slippery. Seated: Desk or counter on an office type chair.
  • Talking: Necessary for communication with residents and giving instructions to nursing staff, nursing assistants, dealing with doctors, family, etc.
  • Hearing: Necessary for taking instructions from physicians and supervisor. Necessary for communicating with residents, Necessary for responding to call lights. Necessary for nursing assessment.
  • Sight: Necessary for doing job effectively and correctly. Assessing physical well-being of residents; observing environment.
  • Smelling: Required for accurate maintenance and detection of wounds, urinary tract
