HIV Peer Navigator Job at San Ysidro Health
Position Summary:
Under the direct supervision of the Coordinated Services Supervisor, with additional direction from the PATH Research Coordinator, the Peer Navigator will provide support to participants living with HIV who are enrolled in the “PATH” (Peers plus mobile App for Treatment in HIV) study funded by the NIH and led by investigators at San Diego State University (SDSU) and UC San Diego (UCSD). The role of the Peer Navigator is to be a bridge between providers and persons living with HIV to facilitate the medical and psychosocial care of the clients. There are three primary functions of the peer navigator: 1) the peer navigator will support their clients by providing one-on-one peer support to improve clients’ retention in HIV care and Anti Retroviral Therapy (ART) adherence, 2) the Peer Navigator will communicate and engage with their clients through the use of a web-based application, or “app” accessed through a smartphone or computer (e.g., using the app to setup notifications and medical appointment reminders for their clients, send and receive direct messages from clients, and monitor clients’ reports of their own ART use and their other activities on the app); and 3) the Peer Navigator will electronically track activities within the app and complete written documentation to regularly report on work-related activities, including participant outreach activities. The Peer Navigator will serve as an empathic, trustworthy, knowledgeable, and reliable resource for the client’s HIV care while maintaining confidentiality at all levels.

Essential Functions of the Job:
- Accompany clients to HIV medical care visits and to other non-medical care visits (if requested)
- Facilitate the clients’ linkage to and retention in HIV care (including reengagement of clients previously lost to follow up)
- Serve as a liaison between the client and the clients’ healthcare providers
- Serve as a key member of the Coordinated Services team, coordinating efforts to support the client with case managers and patient navigators
- Build effective working relationships with clients, case managers, and providers to improve coordination of HIV care and support services
- Record and track client information securely and confidentially
- Problem solve with clients to reduce barriers to care and treatment adherence
- Facilitate client, case manager, and provider communication and understanding of HIV treatment protocols
- Build and support clients’ skills in HIV systems navigation, client-provider communication, and HIV-treatment compliance
- Coordinate client transportation between relevant sites and for the purpose of the PATH program
- Engage in client outreach by searching for clients lost-to-follow-up for study recruitment
- Perform other work-related duties as assigned including occasional weekend or evening work
Experience and Required Skills:
- Familiarity with HIV infection and HIV care in the U.S. and specifically San Diego/San Ysidro
- Familiarity with unique barriers and facilitators to HIV care and medical and social services faced by African American/Black and/or Latinx/Hispanic communities
- Familiarity with the unique health care barriers faced by the LGBTQ+ community
- High School Diploma or GED
- Ability to effectively communicate
- Ability to present in small and large group settings.
- Demonstrate ability to be culturally sensitive and appropriate in working with minority communities.
- Demonstrate respect for diversity.
- Exercise tact, objectivity, sensitivity, strategy and judgment in dealing with a variety of people with a variety of co-occurring disorders.
- Establish and maintain effective working relationships with clients, family members, other professional staff and the public.
- Preferred: Bilingual Spanish/English.
