Job Summary:
- Evaluates and admits patients to Home Care, develops plan of care and provides care to patient according to plan.
- Acts as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs.
- Reviews and plans patient care and or discharge during scheduled case conference meeting and documents accordingly.
- Acts as Liaison for pt/caregiver with MD and community health care partners.
- Strives to lower cost of care through managing mileage, supplies and visit scheduling.
- Completes all documentation related to pt contact; assessments; SOC; Recerts.
- Provides on call coverage for both Home Care and Hospice as assigned
Education:
Associates degree in Nursing and currently licensed in the State of Alabama required.
Experience:
Minimal 2 years experience in a hospital setting working in Med/Surg; Critical Care or ER. Previous Home Care or Hospice experience preferred.
Additional Skills / Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.

Pay: $24.00 - $40.00 per hour
