Mercy Home Health is seeking a part time Home care Scheduler in PA.

Responsible for scheduling clients Home Health/Hospice services with clinician continuity. Identifies client need and schedule clinicians with the appropriate competencies to match client level of care. The Scheduler is expected to exercise judgement in determining the most effective and efficient method of scheduling and rescheduling patients. Provide optimal levels of communication with patients, clinicians, clinical managers, physicians, and homecare coordinators. May also provide coverage for multiple sites and/or Hospice support.

Must be able to work a weekend rotation once monthly .

Opportunity to work from home. Must have reliable internet.**** It is work from home position, but could always change in the future.

Requirements include :

• High School diploma or equivalent
• Health care experience preferred
• Excellent communication and customer service skills.
• Displays optimal critical thinking skills.
• Working knowledge of managed care environment preferred
• Experience multi-tasking on multiple computer systems..
• Must have valid Driver’s license and reliable transportation to and from work site.

• Experience in working with Microsoft products such as Word, Excel, OneNote
