Hospital Liaison Job at Care New England
Summary: Establish self at area hospitals as liaison for The Providence Center and coordinate all hospital discharges for IHH / ACT / CSP teams and outpatient clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Contact area hospitals upon client’s admission and bring status reports to appropriate teams
- Facilitate follow up hospital appointment with licensed provider within 7 days for clients discharging hospital for behavioral health illness
- Schedule appointments for clients with outside agencies if needed
- Coordinate all hospital discharge planning for clients
- Establish productive relationships with case managers, directors and social work departments at local hospitals and regularly visit hospital to establish and maintain relationships
- Establish relationships with team managers and staff
- Meet regularly and at least quarterly with HH and outpatient teams to educate about liaison role
- Communicate regularly with teams regarding discharge plans, special needs, etc
- Promote teamwork to ensure follow up success
- Provide documentation in EMR of all communication activity with hospital and to include notification of admission/discharge to team in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.
- Utilize various EMR reports and QI reports daily that identify client admissions and discharges
- Identify barriers to follow up hospitalization appointments
- Attend various committees as required
- Understand quality measures and the role the liaison plays in meeting the intended target goals
- Perform SBAR with coworker to communicate pending cases to ensure adequate coverage
- Utilize medical information from remote or onsite EMR to access and review medical records
- Give updates to hospital social workers regarding client medication regimen/treatment
- Involve clients, agencies and others in planning
- Present a verbal synopsis of actual cases as required
- Treat clients with respect
- Complete required interagency and other necessary paperwork
- Drive carefully and abide by traffic rules
- Ensure that seat belts are worn by all traveling in Center or employee owned vehicles
- Follow required office procedures in a cooperative manner
- Initiate and process incident reports as per Center guidelines
- Adhere to Infection Control protocols
- Monitor confidentiality in accordance with Center policy and legal requirements
- Work in cooperation with co-workers and supervisory staff
- Communicate with team members regarding problems in a constructive manner
- Attend trainings, case presentations and conferences and other trainings as required

EDUCATION and/or EXPERIENCE
Bachelor’s degree (B.A.) from four-year college or university in Human Services. Experience in Human Services Field preferred.

Must have valid driver's license and insured auto.
