Job Title: Housing Case Manager

This is a full time position.

Duties
Provide culturally appropriate case management services to families and single adults in permanent supportive housing settings, including one sober housing community. This includes intake and assessment, creating individual service plans, documenting client progress, working with other agencies to provide efficient links to resources, providing specialized in-home supports, facilitating ongoing support and/or educational groups, and coordinating services as appropriate. Strong understanding of homelessness, and knowledge of community and educational resources. Experience working with formerly homeless families and with mental illness and chemical dependency preferred. This position will provide services for single adults and families with children.

Job Qualifications

Bachelors degree in Human Service field required. Strongly related work experience of at least 4 years may be exchanged for BA requirement. Minimum one year of related work experience in case management or human service field required. Preferred experience working with clients with chemical dependency.
