HVCS a Division of Cornerstone Family Healthcare is actively recruiting for a Housing Specialist to join our growing team in Hawthorne, NY.
RATE OF PAY/SALARY: $45,000 annually ($24.73 per hour)
WORK LOCATION(S): Hawthorne, NY
STATUS: Full Time
Description of Duties:
- Conduct intake and assessment of clients for emergency financial assistance and short-term housing subsidies.
- Determine eligibility through the timely collection and processing of documentation for emergency financial assistance and short-term housing subsidies.
- Conduct home inspections as needed to determine housing standards adherence.
- Prepare, follow-up and monitor all emergency financial applications and short-term housing subsidies.
- Make referrals for case management, housing, substance use treatment, medical care, legal services, etc. when appropriate.
- Participate in training and program development activities.
- Prepare monthly check requests with all required back-up documentation.
- Data enter program information into the AIRS/eShare system and appropriate Excel tracking spreadsheet.
- Provide information for monthly reports.
- Participate in department and agency meetings.
- Maintain client charts within program standards.
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with Cornerstone policies and procedures and the Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned.
- Engages and assesses client needs to determine program eligibility.
- Completes documentation in a timely, clear and comprehensive manner which is in compliance with agency standards and requirements.
- Provides advocacy and coordination with pertinent service providers to ensure that all client needs are being addressed
- Develops and implements client oriented service plan goals to meet client’s overall needs.
- Arranges outreach with local service providers throughout the seven counties to promote program services.
- Bachelor’s degree in Human Services, or related field with at least one year working with diverse populations and knowledge of HIV issues and services; or Associates Degree with two years of the same experience.
- Knowledge of housing issues and bilingual a plus.
