The Intensive Intervention Services provides intensive and time-limited crisis-stabilization interventions and supports to individuals with mental illness and substance abuse disorders. This stabilization program is intended to respond to Allegany County residents who are experiencing acute symptoms or crisis. Intensive Intervention Services assists adults in obtaining needed medical, social, recreational, educational, financial, vocational and other services through assessment, planning, coordinating, securing and monitoring services.
The assigned program is funded by the New York State Office of Mental Health (OMH) as part of reinvestment dollars and is required to adhere to all OMH regulations and guidelines and the US Department of Health and Human Services.
Duties: Serve as Primary Coordinator: Review the SPOA application, supporting documentation and consult with the SPOA coordinator.
- Contact the referral source for further information, as needed • Within 24 hours contacts the individual to schedule an initial face-to-face contact in their home or community location.
- Refer to or link client to other community-based service providers as an alternative to inpatient evaluations and/or hospitalization, as indicated by the family and needs assessment.
- Obtain releases of information for all services that the individual is involved with.
- Complete a rehabilitation assessment that will identify client needs, risks, supports, cultural information, and religious information. This will provide the groundwork for work with the individual.
- Responsible for gathering collateral input into the assessment, action plan and WRAP plan.
- Develop an individualized service plan with the client and implement within 14 days of referral to the program. The Service Planning process is recovery-oriented, building upon the client’s strengths to help meet his/her needs.
- Conduct a Service Plan Review at 4 weeks of service in order to monitor the client’s progress toward treatment goals and to determine the continued need for service provision. It is anticipated that, through specialized support and linkages, the client will have achieved a reasonable stable level of functioning within 6 -10 weeks.
- Responsible for providing or making arrangements for necessary medical, dental, mental health, financial, legal, social, educational, psychological and/or other assessments, as needed for the client and/or family as identified in the assessment process. Utilize WRAP around funds to address needs as appropriate.
- Discuss relevant cultural, religious, ethnic, racial, developmental, age, social, sexual orientation, geographic, and other factors that influence and play a role in the client’s life.
- For clients needing services beyond 6-week period, submit an Extension Request
- Be aware of and adhere to HIPAA regulations and agency’s confidentiality and record keeping policies and procedures.
- Coordinator will be placed on the crisis hotline rotation schedule to provide crisis coverage.
- Perform all necessary discharge and termination functions for individual clients as needed within one month of discharge/termination.
- Maintain contact with other service providers based on individual needs of client and family.
- Must participate in clinical team treatment environment and adhere to established clinical rehabilitation theories and practices unless ethical conflict occurs.
- Report any ethical conflict to supervisor or director.
Requirements:
- Master’s degree in social work, counseling, psychology or related field with credentialing by the NYS Department of Education (DOE), NYS Office of Alcoholism and Substance Abuse services, American Association of Pastoral Counselors, or credentialing by other organizations sanctioned by the Office of Mental Health.
- Two years’ experience in mental health or other human services, working with adults preferred.
- Strong computer abilities and proficiency with electronic medical records (EMR).
- Professional appearance and demeanor.
- Valid background clearances with NYS Justice Center and Office of Child and Family Services.
- Valid driver’s license.
