Constellation Health Services is looking for compassionate and empathetic Licensed Clinical Social Workers to work with our hospice, palliative & transition support patient needs. This role will involve providing care in the patients' homes and at Long Term Care Facilities on a per diem basis with growth into a full time role

Territory: Houston, Texas ( Conroe, The Springs and Woodlands)

This Is You:

• LCSW in the State of Texas with at least 1 year of clinical experience in Acute Care, and/or Counseling preferred
• Have reliable transportation and willing to commute within a designated territory
• COVID Vaccinated or qualified exemption
• Strong interpersonal and written communication skills.
• Ability to be self-directed and work independently.
• Thorough knowledge of current computer software (ie. Word, Excel, 365)
• Able to work within a team environment
• The ability to work effectively in high-activity situations and under pressure.
• Willing to work a rotating on-call if needed
• Some weekend days are required, and summer and winter holiday commitments as needed

Together, we:

• Provide Clinical Social Work to Hospice, Palliative &/or Patients needing support post facility discharge for transition support.
• Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plan intervention based on evaluation findings
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame
