We are currently hiring for a LPN in our Jamestown/Fredonia Area! As a LPN at Venture Forthe, you will be responsible for the following:

• Provides care to client’s in accordance with the NYS Practice Act.
• Implements medical orders and established plans of care.
• Follows agency policy and procedure.
• Obtains information from the client regarding complaints, status of illness, problems obtaining services from other sources, problems with supplies and equipment, communicating to the DHC as appropriate.
• Performs medication review and accountability.
• Assists the physician and/or Registered Professional Nurse in performing specialized procedures.
• Evaluates the appropriateness, cleanliness, and safety of durable medical equipment, supplies, and/or appliances.
• Assists the client in learning appropriate self-care techniques.
• May perform aide supervision, as directed.
• Documents visit notes and progress notes, to reflect services/client activities
• Communicates with other members of the health care team.
• Adheres to established policies and procedures.
• Participates in the agency’s CQI/PI program as requested
• Attends staff meetings. Participates as a member of committees, as requested.

Qualifications

QUALIFICATIONS:

• NYS Licensed Practical Nurse
• Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to speak effectively before customers/patients or employees of the agency.
• Excellent organizational writing, communication and interpersonal skills.
• Ability to apply common sense understanding to carry out instructions furnished in written, verbal or diagram form.
• Required to participate in on-call rotation, position works alternate weekends.
• Home Care experience is a plus, but not required.
