We are looking for a Home Health LPN. Pay is starting from $72,800 annually. Rate is negotiable depending on experience. We are looking for coverage on the west side! We offer benefits (Medical, Dental, Vision) and mileage reimbursement for our Full time employees.
Duties:
Implements plan of care initiated by the clinician.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the clinician and/or clinical manager.
Preferences
- Current Licensed Practical Nurse
- One (1) year of experience working under your LPN license
- Current CPR with BLS certification
- A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
