The LVN Employee Health & Wellness Coordinator (LVNEHWC) works within a multi-disciplinary health care team in the process of organizing, tracking, and coordinating the Employee Initial and Annual Health Screening and Employee Health & Wellness programs. This position provides nursing assessment, executes treatment orders, promotes health education and develops wellness support plans for the employee and non-employee/volunteer population at Valley Community Healthcare within the areas of the pre-employment clearance, annual health examinations and immunizations, employee wellness program, employee illness, and work injury/illness. Under the supervision of the Nurse Manager (NM), and in coordination with the Director of Nursing (DON) and the Chief Medical Officer (CMO), the LVNEHWC performs a wide variety of nursing and administrative duties in support of in-coming and current Valley Community Healthcare employees and non-employee/volunteer populations to ensure compliance with Federal, State, and agency requirements with regard to employee health and wellness

CORE JOB RESPONSIBILITIES (Essential Duties):

· Collaborates with Human Resources staff, Credentialing Coordinator/Risk Manager, and the Nurse Manager to maintain a roster of newly hired/current employees and non-employees (students, trainees, interns, residents, or volunteers), hereafter referred to as healthcare workers (HCW).

· Ensures that all HCW complete required health examinations, immunizations, and screenings prior to starting work at VCH.

· Ensures that all HCW complete annual examinations, immunizations, and screenings.

· Communicates with all HCW regarding available options for completing examinations including both VCH and non-VCH providers.

· If a HCW requires that their pre-employment examination, immunization, and screening be completed at VCH, the LCSW will follow scheduling protocols as established by the NM, DON, and CMO in order to ensure that the appointment provided does not disrupt VCH patient schedules.

· Communicates with Human Resources staff when the start date new hire must be delayed due to schedules for appointments.

· Follows VCH protocols for documentation of external immunizations by accessing the California Immunization Record (CAIR) and/or paper vaccination records.

· Reconciles external records to ensure VCH electronic health records reflect correct immunization and screening status.

Following physician or midlevel orders, administers immunizations, executes lab orders, places PPD skin tests, provides initial and annual health assessment forms

· Maintains well-organized and up-to-date record keeping systems while ensuring confidentiality of HCW health records.

· Contacts, documents, and follows up with HCW to ensure adherence and timely completion/administration of annual health clearance and required immunizations.

· Accurately and completely documents all activities in appropriate employee records system.

· Administers vaccines safely and in accordance with vaccine schedules.

· Tracks and provides reports to the NM regarding the status of annual immunization and tuberculosis testing for HCW.

· Up to date knowledge of COVID-19 protocols regarding testing, isolation versus quarantine recommendations, and vaccine timing, handling.

· Assists the NM, DON, and CMO with reports on HCW who are sick or need to be off of work due to emergencies.

· Communicates with the NM, DON, and Chief of Human Resources (CHRO) regarding workplace related injuries and follows protocols to ensure HCWs receive prompt attention for such injuries.

· Follows established incident reporting protocols and submits timely reports.

· Participates in interdisciplinary team conferences.

· Maintains logs and statistics as needed. Prepares reports or conducts audits as required.

· Participates in the VCH Quality Improvement process.

· Conducts in-house or outreach activities as needed.

· Assists with the implementation of clinic policies and procedures as needed within the health center working in concert with other staff to resolve interdepartmental issues, recommending changes in policies and procedures as needed.

· Participates in all safety programs, which may include assignment to an emergency response team. Participates in hazardous waste and infection control assignments as required in the health center which may include being designated as an emergency responder to a hazardous substance release or spill.

· It is the responsibility of every employee to report any patient safety concern to their immediate supervisor without fear of reprisal. VCH policy of non-retaliation protects employees who report patient safety concerns.

· Performs other duties as required or as assigned by Supervisor.

CLINIC RESPONSIBILITIES:

  • Acts as a clinical resource to staff
  • Thorough knowledge of subcutaneous and intramuscular injection techniques, phlebotomy and assessment of PPD skin tests.
  • Excellent for EPM and EMR with NextGen computer systems
  • Assists with vaccination inventory, oversight of immunization logs, and participates in clinic audits as assigned.
  • Responsible to ensure vaccines are administered safely.
  • Participates with all regulatory audits in conjunction with DON and Nurse Manager, and Site Supervisors.
  • Routine operational and support functions; directing patients to appropriate departments, provide staffing to other areas as needed, cover in-clinic when short-staffed.
  • Participates with all regulatory audits in conjunction with DON, Nurse Manager, and other departments as necessary
  • Forward USPS/UPS/Federal express package information to the appropriate area in a timely manner
  • Ensure encounters/corrections from department are verified and delivered in a timely manner to billing department
  • Participates and assists in appropriate staff meetings, required in-services, and training

POSITION QUALITFICATIONS:

These are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this job. Individual abilities may result in some deviation from these guidelines. To perform effectively in this position, the candidate must have:

· 3 years experience in a community based clinic environment is preferred

· Must hold a current Licensed Vocational Nurse License in the State of California

· Current BLS card

· Bilingual in English and Spanish preferred, with excellent written and oral communication skills.

· Minimum of one year working in a community clinic or other medical office environment is required.

· Possess the ability to work independently and as team member.

· Possess excellent organizational and problem solving skills and have the ability to prioritize.

· Have current CA Driver’s License, and available vehicle, and valid liability insurance.

· Experience with basic computer skills including word processing and data entry.

· Experience with electronic health records preferred.

· Must have excellent customer service skills as well as people skills
