Looking for a full-time LifeSet Specialist.
Job Summary:
The LifeSet Specialist is responsible for providing services to address the needs of young people who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement.
What You Will Do:
- Provide individual services to 8-10 young adults in the home and community setting to assist young adults with the transition to adulthood.
- Meeta minimum of one time per week face to face with the young person and provide additional support via phone, text or in person as needed.
- Directly responsible for all interventions associated with a specific young adult and any additional resources or supports.
- Support a young person to identify goals and work with them to achieve them.
- Conduct ongoing assessment of the young person's needs from a strength-focused, solution-based perspective and develops service plans monthly.
- Document all service contacts with the young adult within the timelines required by the organization.
- Be on call rotation and respond in person at any time when it is determined to be necessary by the Supervisor, LifeSet Model Expert and Program Director.
- Attend and actively participate in trainings and individual and team supervision and development and to follow up and/or implement feedback and recommendations received from the Supervisor, Program Director and LifeSet Model Expert.
- Adhere to all ethical and legal guidelines, including confidentiality and mandated reporting.
- Work will be in Bergen, Passaic, Hudson, Morris and Essex county.
What We Need to See:
- Bachelors degree in social services with one-year related experience in counseling and or case management or other related experience is required OR Masters degree in field of social work, psychology or related field preferred.
- A valid driver's license and must be insurable under Agency automobile insurance.
- Ability to maintain a flexible schedule.
- Bilingual in Spanish preferred.
