POSITION OVERVIEW
Hospice of the Piedmont is seeking a full-time LPN – Critical Care Response Nurse to work rotating nights, evenings, and weekends in Charlottesville and surrounding counties. By collaborating with our Triage Nurse our LPN – Critical Care Response Nurse will respond promptly to the patient’s bedside and provide a high standard of care for the patient’s and family’s needs.

DUTIES include but are not limited to:

• providing home visits to assess patient needs
• providing education to the patient and their family
• performing nursing evaluation visits to new admissions and determining eligibility based on agency criteria
• consulting with physicians, staff, and vendors
• observing and reporting changes in patient’s status and needs
• implementing appropriate interventions to ensure the final chapter of your patient’s life is one of quality, compassion, and dignity.

QUALIFICATIONS
Our LPN-Critical Care Response Nurse should have:

• Graduated from an accredited nursing school and possess a valid LPN-Registered Nurse license in the Commonwealth of Virginia
• Two years of relevant nursing experience
• strong communication skills;
• the ability to work independently and within a team;
• the capability to manage all aspects of terminal care including coordination of services and documentation;
• emotional maturity and the capacity to provide compassionate support to terminally ill patients and their caregivers.
• the ability to make well-thought-out decisions in stressful situations is needed.
• Demonstrated ability to respond to the needs of patients and caregivers in varied settings

SALARY
Starting salary for the LPN - Critical Care Response Nurse is $60,000 annually.
