PACE, a member of Empath Health is currently seeking an LPN Home Care Coordinator to join our team.

The LPN Home Care Coordinator:

• Under the supervision of the PACE Senior Staff Nurse, assesses and coordinates the home care needs of a frail elderly population, and identifies and develops specific plans of care in combination with the IDT.
• Provides nursing care and teaching to participants, families and caregivers.
• Performs assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.

Position Requirements

• Education and/or Experience: Must be a graduate of an accredited school of practical nursing and possess a current Florida license to practice nursing. Prefer applicant have at least 18 months LPN experience and a minimum of one years’ experience working with the frail and elderly population.
• Licensure:
• Licensed LPN in the state of Florida
• Valid CPR certification from the American Red Cross or American Heart Association.
• Must have a valid driver’s license, proof of insurance and have means of transportation.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
