• Provides direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care in collaboration with patients/CG’s, Health Care Provider, case manager and or Supervisor as assigned and documents appropriately.
• Observes and reports the need for additional services and equipment on an ongoing basis and informs case manager and/or supervisor in a timely basis.
• Observes and reports on the home environment and make recommendations that will enhance patient care, promote safety and prevent adverse effects.
• Works with patient, family and health care team members effectively to coordinate services, promote quality of care and positive patient care outcomes. Updates supervising RN/Nursing Supervisor and care team on progress toward goals for plan of care and implements changes/additions to the POT in collaboration with patient/CG as needs change and goals are met.
• Takes verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately.
• Coordinates plan for patient care outcomes through case conferences with supervising RN and other team members to provide appropriate, effective and efficient patient care, discharge planning and documents appropriately.
• Informs Health Care Providers of changes in patient’s condition in accordance with providers orders and documents communications accordingly.
• Provides care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions. Provides instructions to patient/CG’s and staff in a timely manner.
• Attends case conferences, in-services and staff meetings.
• Demonstrates professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues, as well as, updates to patients’ clinical records including medication profiles, plan of care, case conference and progress notes as required in a timely (24hrs) manner.
• Cooperates and is reliable with making PRN, weekend and holiday visits to patients as assigned.
• Informs supervisory personnel of any patient change in condition and or incidents promptly and documents appropriately.
• Uses appropriate teaching materials and techniques to obtain positive outcomes and documents patient/CG response to education.
• Provides and maintains a clean safe working environment through observation of infection prevention & control and safety policies.
• Provides and documents services in accordance with state, Medicare, Medical regulations and agency policy as applicable.

• Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

• Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse and CPR certified.
• Has at least one year clinical practical nursing experience. Home Health experience preferred.
• Must have vehicle, current driver’s license and automobile insurance.
• Interpersonal and communication skills necessary.
• Ability to make home visits and provide skilled nursing assessments and care in the home.
