GENERAL JOB SUMMARY:
Oversees and facilitates the completion of resident assessments in the skilled nursing facility. Throughout the assessment and while planning care, ensures compliance with Federal and State regulations. Incorporates the Generation’s mission, vision, and values in the daily work and interactions with others.
SCOPE OF JOB TASKS:
Duties include but are not limited to the following:
- Administer patient assessments.
- Oversee the assessment process
- Set the assessment schedule and assuring that assessments are done in an accurate and timely manner.
- Coordinate care plans according to regulatory requirements.
- Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
- Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator.
- Complete and assure the accuracy of the MDS process for all residents.
- Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
- Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement.
- Facilitate problem-solving for complicated admissions.
- Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
- Assists personnel and residents in establishing a home–like atmosphere.
- All other duties assigned.
JOB SPECIFICATION:
Education:
- Current LVN or RN in good standing in the state of CO
- Certification in CPR and First Aid.
- Ability to read, write, communicate, and follow written and oral instructions in the English language.
Work Experience:
- Clinical experience in a healthcare setting
- Demonstrates the knowledge of state and federal regulations
Job Type: Full-time
Pay: $37.00 - $48.00 per hour
