We are excited to announce: SUMMER COMMONS, a new licensed 64-bed skilled nursing care center combined with a 30-bed assisted living unit specializing in Memory Care. This NEW, state-of-the-art health care center located in Sanford is seeking an enthusiastic MDS Coordinator. Duties will include managing the entire MDS process, completing assessments and developing care plans.
The ideal candidate will have: * A friendly, team-oriented disposition * Flexibility, and be detailed oriented * Understanding of PDPM * Experience in a long term care or rehabilitation setting * RN required * Experience with the MDS is preferred but not required * State of Maine License *Must be able to meet requirements of the Maine Immunization of Health Care Workers statute.* https://northcountryassociates.com/career-opportunities/apply-online/
