  • Develops a care plan that specifies goals and actions to address accessing the medical, social, educational, and other services needed by the individual.
  • Provides ongoing monitoring over service provision to ensure the client is receiving the services identified on the treatment plan.
  • Reviews client history and identified needs from initial evaluation/intake form and treatment plan, identifies client needs, and completes related documentation.
  • Gathers information from sources, such as family members and medical providers to form a complete assessment of an individual.
  • Engages in regular contact with active and non-active OCK clients a minimum of once a month.
  • Assists OCK clients with PCP, dental, and other healthcare providers, including finding a provider if they do not have one.
  • Assists in obtaining affordable medications and procuring DME equipment as needed.
  • Teaches and encourages self-care for both mental and physical health, including diet, exercise, movement, and interpersonal boundaries.
  • Assists OCK clients in scheduling and keeping medical appointments.
  • Identifies and addresses any medical concerns that may arise.
  • Completes referrals to other providers as needed.
  • Receives and manages incoming reports and referrals from three managed care organizations (MCOs).
  • Attends ongoing One Care Kansas meetings.
  • Contacts referrals by phone and/or in person to complete HAP and sends completed HAPs to nurse to sign.
  • Tracks referrals, caseloads, opt-outs, refusals, and discharges. Keeps a record of contacts for billing purposes.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and CrossWinds standards and requirements. Submits all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements.
  • Develops knowledge of community resources and how to access these services for the benefit of clients.
  • Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, agency staff members, and visitors.
  • Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements.

Work Environment

This job operates in a community setting, as well as in a professional office setting. This position primarily works with clients out in the community and within the agencys residential facilities. This role occasionally uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; handle objects; grasp and reach for items with hands and arms; manipulate hands and fingers; climb stairs; balance; stoop, kneel, or crouch; and talk or hear. The employee must occasionally lift or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Language skills are required, including the ability to read, write, comprehend, and speak the English language. This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.


Position Type/Expected Hours of Work

This is a full-time position. Monday Friday, 8:00 am 5:00 pm.

Required Education and Experience

Associate Degree plus 2 years experience in the health and human services field.


Preferred Education and Experience

Bachelors Degree in psychology, social work, counseling, or related field.


License or Certification

Must complete Case Management Training and provide training certificate to agency within six months of hire.

Must complete One Care Kansas training.


Additional Eligibility Qualifications

Ability to work with persons with severe and persistent mental illness; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to work independently and as a member of a therapeutic team; possess prior computer experience in Microsoft Office programs and basic computer fluency.
