Position Description

The role of the Medical Records Associate is absolutely critical for effective, accurate and excellent patient care. Accuracy and efficiency of the medical records team directly impacts our mission to be the leader in physician-managed healthcare services and to provide excellent surgical and clinical care for every patient, every day.

Key Duties & Responsibilities

The key duties and responsibilities of the Medical Records Associate include, but are not limited to:

Scanning

  • Scan and upload all patient record documentation in a timely manner to facilitate timely coding and billing and to accurately account for all patient information
  • Scans, files and imports to the electronic health record new and established patient paperwork including all documents generated for all patients during the appointment
  • Monitors and scans upcoming referrals received via fax or US mail, scans and imports to the EHR in a timely manner

Access and Retrieve

  • Locate and access archive files
  • Responsible for gathering/retrieving protected health information (PHI) of all patients treated by a clinical provider
  • Retrieve all necessary chart documents for patients receiving services. This includes but not limited to Patients logs, dictations and addendums, provider and nurse progress notes, discharge summaries
  • Assists retrieving archived paper charts from storage for upcoming appointments
  • Assists pulling, scanning, importing, and filing paper charts into the electronic health record EHR)

Abstract and Track information from charts

  • Abstract information from charts into the EHR
  • Track chart notes and dictation
  • Track internal documents for all patients; these internal documents may include detailed plans, progress reports, assessments, and other requirements
  • Track approval paperwork, request modifications or additional paperwork, as requested.
  • Communicate deadlines and status of paperwork with department supervisors

Prepare and Respond

  • Prepare and release medical records as you follow HIPAA guidelines
  • Facilitate requests from other departments
  • Act as a liaison between the care center/facility and outside entities that either request paperwork or who submit requested paperwork to the center
  • Request required approval paperwork from internal and external service coordinators.
  • Send reports to external service coordinators and document required submissions for the care center/facility
  • Complete duties and assist others as directed
  • Follow all policies as introduced during new associate On-Boarding, updated real-time and outlined in Associate Handbook

Experience, Skills and Abilities

  • High School Graduate or the equivalent required
  • Minimum on six months experience in the healthcare industry.
  • Preferred Medical Record Training and basic Medical Terminology understanding
  • Previous work experience in customer service, medical records, information technology or other related health care field in a plus
  • Comfort working in healthcare computer systems
  • Knowledge of computerized information systems
  • Experience with Epic EHR and Nextgen EPM is a plus
  • Keen attention to details

Work may be performed in an office and clinical environment. Requires corrected vision and hearing to normal range. While performing the duties of this job, the associate is regularly required to talk or hear. The associate is required to sit for long periods of time, stand and walk, bend and stretch. Use of telephone and computer is required. Manual dexterity required for use of computer keyboard. Occasionally lifts and carries items weighing up to 40 pounds. May requires working under stressful conditions or working irregular hours.
