Responsibilities:
Position Summary
A Medical Scribe provides documentation assistance to a physician focusing on timely and accurate completion of medical records, within the scope of his or her certification or licensure, if applicable.
Actively listens and is able to extract all pertinent information from conversations pertaining to patient care between patient and physician.- Assists with taking notes, documenting words and activities of the provider in real time during the patient encounter and accurately entering within the medical recor
- Supports physicians by reading the medical record and summarizing care in the patient note as well as summary of care to be reviewed and authenticated by the physician.
- Ensures signature, including name, title, date and time, is consistently completed for all entries in the medical record. Clearly identifies and distinguishes scribe's
- Receives timely documentation authentication from physician per guidelines. Ensures authentication takes place with physician before leaving patient care area.
- Strictly follows all Joint Commission and CMS, State and hospital policies and procedures regarding documentation standards.
- Performs other duties as requested.
Qualifications:
Minimum Qualifications
- High School diploma or equivalent required
- Completion of Medical Terminology course
Preferred qualifications:
- Bachelor's degree in clinical discipline
- EPIC experience
