Medical Social Worker/Medical Care Coordinator (RW)

Western NC Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC). We are looking for a full-time Medical Social Worker/Medical Care Coordinator to coordinate healthcare services and assess and assist with addressing social determinants of health that may serve as barriers to achieving positive health outcomes among women and children enrolled in our Ryan White services for those living with HIV/AIDS.
RESPONSIBILITIES- Includes but not limited to the following:
- Assist patients with accessing emergency funds and other resources available to enrolled patients
- Connect patients and coordinate care between primary care, HIV care, behavioral health, dental, and pharmacy
- Provide case management services to address food insecurityand coordinate access to healthy foods by assisting with access to community-based food and nutrition resources
- Provide housing navigation and support servicesand assist individuals with developing a housing support plan
- Connect patients with necessary transportation services
- Connect patient with other appropriate community resources and social supports as necessary
QUALIFICATIONS:
- Bachelors’ degree in human services, social work or psychology, Master’s degree preferred
- Previous experience in care/case management
- Preferred experience with patients living with HIV/ADS
- Expert knowledge of medical office operations
- Familiarity with role of FQHCs in the community
- Valid, unencumbered driver’s license required

Pay: $20.57 - $22.73 per hour
