The Mid-Level Provider will be responsible for providing medical care consistent with accepted professional standards, including examinations, treatment, referral and related documentation. This position will include primary care services and walk-in primary care services.

Job Duties:

• Provides guidance and leadership as an integral participant of a care management team.

• Provides primary care services within the scope of mid-level training for clients of the agency.

• Assists in the coordination and/or supervision of preventive services, diagnosis, treatment, medical and support services and documentation.

• Communicate patient treatment plans with appropriate staff members

• Prescribe medications as necessary.

• Carefully document all medications, diagnoses, treatments and consultations.

Job Qualifications:

• Graduated from an accredited physician assistant or nurse practitioner program.

• Current active licenses, in good standing, to practice as a PA or NP in the State of Michigan.

• Must maintain Board and BLS/CPR certifications.

• Insurable for liability coverage.

• Passion to work with the underserved population.

Job Type: Full-time

Pay: $79,760.43 - $159,022.08 per year
