The Nurse Case Manager will provide help to high risk infants and children getting health care and developmental services. The Nurse case manager will complete the following:
- promote optimal growth and development of infants.
- provide emotional support and relevant referrals to families for essential services.
- work with the mother to reduce stress and help explain identified medical conditions for further evaluation, diagnosis and treatment.
- responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High Risk Infant follow Up Program from the State of Illinois.
Requirements
- Bachelor Degree in Nursing from an Accredited College or University
- Minimum of 2 years' experience working in Community Health or Maternal Child and Health nursing
- Active nurse License in the state of Illinois
Job Type: Full-time
Pay: $27,280.75 - $66,959.65 per year
