The Orthopedic Nurse Navigator is responsible for guiding orthopedic patients throughout the entire continuum of care, from office visit to surgical procedure, discharge to home/Skilled Nursing Facility (SNF), and provide follow-up/guidance throughout the post-discharge phase. The focus of this role is to streamline the services provided by the Center for Orthopedic Excellence, enhance the patient/family experience, and improve patient quality measures and outcomes and decrease the LOS. The Orthopedic Nurse Navigator is a critical member of the Center for Orthopedic Excellence Team, Orthopedic practice managers, Surgeons, Home Health Agencies, SNFs, and Community programs to effectively coordinate the plan of care prior to the day of surgery and beyond discharge. Some travel may be required.

Full Time 32 Hours Per Week

Shift: 7:00 am to 3:30 pm

Key Responsibilities:

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

  • Communicates effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed.
  • Develops and maintains cooperative relationships with the Center for Orthopedic Excellence Team and other relevant members of the health care team.
  • Coordinates, supports, and provides guidance for the orthopedic patient and family throughout all phases of care.
  • Engages patients and their families at the Patient Education Program.
  • Engages with the quality department and facilitates the performance improvement data collection and tracking processes for core measure process improvements.
  • Regularly meets with physician practices and their office staff to maintain ongoing communication and follow-up.
  • Attends Center for Orthopedic Excellence Team meetings and provides program updates.
  • Updates best practices and clinical practice guidelines with the American Academy of Orthopedic Surgeons CPGs
  • Develops, coordinates and oversees the COE yearly education plan and participates in the COE staff education
  • Champions nursing certification for Orthopedic and surgical nursing within the COE.
  • Serves as an advocate for the patient during the development and implementation of the plan of care.
  • Round on all Total Joint patients post-operatively to enhance patient satisfaction and ensure quality of care.
  • Participates in community education and events related to the Center for Orthopedic Excellence.
  • Maintains accurate and confidential records documenting numbers of patients served, services provided, and unmet needs.
  • Maintains patient confidentiality in accordance with HIPAA regulations.
  • Performs other duties as assigned by the Director, as needed.

Required Knowledge and Skills:

  • Excellent verbal and written communications, as well as listening skills.
  • Working knowledge of Microsoft Word, Excel and Power Point.
  • Ability to prioritize workload, handle multiple tasks, and to work independently with limited supervision.
  • Detail oriented and focused.
  • Strives for high quality performance and takes initiative to make improvements and implement creative solutions.
  • Ability to work in a team environment and to develop and maintain collaborative relationships with patients and their families and all members of the healthcare team.

Education/Experience/Licensure/Technical/Other:

Education:
  • Bachelor's Degree in nursing required
Experience:
  • Experience in Orthopedics required, minimum of 3-5 years preferred.
Certification/Licensure:
  • MA Nursing License, BLS required. Orthopedic nursing certification preferred.
Software/Hardware:
  • Strong computer proficiency required, including Meditech, Microsoft Office products, Databases, and Internet.
Other:
  • Must demonstrate excellent organization and multi-tasking skills.
  • Must demonstrate professionalism and possess the skill, knowledge and ability to interact collaboratively with healthcare teams.
  • Demonstrates innovation, expert problem solving skills and is self-motivated.
