Looking for intelligent, kind hearted NP/PA for distinctive Sub-Acute Rahabilitation and Hospital facility
We are looking for a PART TIME (M,We,Fri) or FULL TIME, Nurse Practitioner or Physicians Assistant to work with a Transitional Care Team in a Post Acute facility in North Bergen County. Job responsibilities include treating patients with complex medical problems with goal to prevent readmissions and transition patients back to the community. Experience with hospital or post acute care highly desirable but a strong background in Med/Surgical nursing would suffice even if it is first job. Practitioner will be a part of a comprehensive care team.
Job Types: Part time or full time
Salary: $70.00-$75.00 per hour for part time, or 130-150/ year full time
