Summary

Under the supervision of the Clinic Manager, provides occupational therapy services to the participants of PACE program.

Essential Duties

· Performs initial and annual screenings/assessments as assigned and makes appropriate recommendations for services with special focus on improving or maintaining independence and safety.

· Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participation in, interdisciplinary team meetings; collaborates on care planning and service allocation decisions.

· Completes participant evaluations as needed and develops a plan of care. Areas evaluated include but are not limited to: functional, physical and activities of daily living (ADL) status, cognitive functioning, visual-perceptual status, and feeding ability, cooking skills, bed mobility, wheelchair needs, home safety /environmental assessments and restraint assessments.

Education

Bachelor of Science degree in Occupational Therapy from an accredited program, and current certification with State of Oregon.

Experience: A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly population required.

Skills and Knowledge:

· Effective written and oral communications skills.

· Effective problem-solving skills.

· Productive as both and independent and team contributor.

· Well organized; demonstrated ability to effectively prioritize, plan and execute responsibilities.

· Effective computerized charting skills.

· Demonstrates necessary skills and knowledge as outlined in position-specific Competency Assessment Profile.
