The Patient Representative is charged with assisting their assigned primary care physicians in growing and maintaining full patient panels for comprehensive primary care, managing clinic schedules, communicating with patients to facilitate timely planned follow-up appointments, re-engaging with patients who miss planned appointments, and providing access for acute care. The Patient Representative assists the primary care physician in all aspects of follow up to meet appropriate chronic disease and preventive care metrics for quality and participates in quality improvement and metrics reporting for the assigned physicians. Required Knowledge, Skills, and Abilities
  • Ability to use MS Office, Excel, Adobe, IDX, Meditech, eClinical Works, Epic
  • Excellent customer service skills
  • Ability to perform in a culturally diverse, patient centered, holistic atmosphere.
  • Ability to effectively interact with patients, as well as internal and external constituencies
  • Ability to promote and build teamwork and multidisciplinary care concept
  • Ability to efficiently interpret and follow oral and written instructions
  • Ability to work on multiple tasks with multiple groups independently and concurrently and prioritize tasks to accomplish work completion successfully
  • Professional attitude and appearance
  • Strength in multitasking
Required Education and/or Experience • High School Diploma or equivalent
  • Two years of related work experience
Preferred Qualifications • Bachelor’s Degree
  • 3 years work experience in clinic or administrative setting
  • Proficient in the review of medical records and other patient care information
  • Knowledge of Medicaid, Medicare, and other insurance precertification procedures and requirements
  • Basic understanding of health, prevention, and disease systems
