The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides physical therapy services to patients accepted for care by RVNAhealth as prescribed by the attending physician. The therapist is responsible to the Director of Rehabilitative Services for fulfilling the needs of the agency in the area of his/her expertise. This is a weekend position.

Job Responsibilities:

  • Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan with RN case manager, patient, family and physician.
  • Oversees patients as needed. No case management.
  • Identification of patient and family needs for other home health services and referral for same when needed.
  • Maintains clinical progress notes which include continuing assessment treatment (s) results noted and communication with other personnel.
  • Serves as a consultant to other health team personnel regarding physical therapy for individual patients.
  • Assists patients to reach a level of function to enable them to remain safely in their home environment.

Qualifications & Experience:

  • Graduate with a baccalaureate degree of an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education with collaboration with the American Physical Therapy Association.
  • Current license to practice as a Physical Therapist in Connecticut.
  • At least two (2) years' clinical experience. Preferred Home Health Experience.
  • Ability and willingness to follow agency policies and procedures and works as a team member.
  • Strong Communication Skills
  • Reliable transportation and the ability to drive in all types of weather conditions.
