The Primary Therapist II leads the social work team in the absence of his/her supervisor and coordinates the psychosocial services in the area of need. As a member of the clinical treatment team the Primary Therapist II provides care to patients utilizing a variety of modalities. This position coordinates the care and management of patients and family members as well as bringing cases to final disposition and is primarily responsible for development and implementation of social work programs in their assigned ambulatory and/or inpatient unit.

Responsibilties:
A Day in the life of a Primary Therapist at HMH/Carrier Clinic includes:


1. Provides a comprehensive assessment, inclusive of evidence based validated outcome measurements of patient's psychosocial needs and develops treatment plans and treatment plan reviews based on findings and according to department policy;
2. Provides effective treatment to patients, whether assessment or therapy (individual or group), and shares and interprets information to staff, families, and/or other approved agencies.
3. Provides psychotherapeutic services to patients in accordance with Licensure regulations defined by the Board of Social Work Examiners and obtains and interprets information in terms of patient needs, knowledge of growth and development, and an understanding of the range of treatment required for that age group.
4. Assists in preparation, coordination and evaluation of social work policy and procedure.
5. Assists with interview and orientation of new staff members.
6. Oversees the clinical area of social work interns and one staff member and complete performance evaluations, if applicable.
7. Assists in the developing and evaluating quality assurance and performance improvement programs.
8. Attend committee meetings and complete administrative tasks such as statistical analysis of patient volume.
9. Completes written reports in a timely fashion as required.
10. Provides comprehensive case management services and psychosocial assessment.
11. Develops a comprehensive discharge plan, in collaborations with the patient/family patient caregiver, patient support persons and healthcare team that will provide maximum benefit for each patient as needed.
12. Ensures that the discharge plan will be the least restrictive environment that best meets the continuing care needs of the patient.
13. Ensures provisions of continued care at home or in an appropriate extended care facility based upon the patient needs.
14. Confirms the patient has a primary care provider upon discharge or refers appropriately to an FQHC.
15. Reassesses periodically and evaluates against care goals and the plan of care and, revises when indicated. Medical records reflect that each patient's discharge plan is re-assessed no less than weekly in response to change in medical situation.
16. Complies with unit practice standards relating to case management including addressing family and community resources, eligibility for entitlement programs, and other health care providers.
17. Provides instruction to patients and families related to illness and expectations of treatment addressing their learning needs and learning style.
18. Integrates discharge planning into ongoing treatment to assure a continuity of care for patient upon completion of treatment.
19. Functions collaboratively with treatment team members to enhance service to patients.
20. Maintains patient records as required by law and Medical Center policy.
21. Assists other mental health professionals in expediting emergencies and/or functions as co-therapist in special situations.
22. Delivers and documents all care in an efficient and productive manner.
23. Has a working knowledge of various community agencies and resources and refers patients when appropriate.
24. Presents at professional meeting locally and nationally and prepares materials for publication in professional journals, if applicable
25. Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 4 hours a day.
26. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
27. Adheres to the standards identified in the Medical Center's Organizational Competencies.


Qualifications:
Education, Knowledge, Skills and Abilities Required:

1. Master's degree in Social Work (MSW) or in Counseling.
2. Minimum of 5 years of post MSW experience.
3. Proficiency with policy review and program planning.
4. Demonstrates leadership abilities
5. Trained in individual and group psychotherapy techniques.
6. Knowledge of psychopathology and differential diagnosis and treatment techniques.


Education, Knowledge, Skills and Abilities Preferred:

1. National Association of Social Workers Clinical Social Work Supervision Certification Course.


Licenses and Certifications Required:

1. NJ Licensed Clinical Social Worker or Professional Counselor License.
