Hummingbird Hospice is seeking qualified and motivated PRN Registered Nurse to join our Hospice team. Each staff member must be committed to the hospice philosophy and mission and provide outstanding customer service to both internal and external customers. PRN RNs work under the direction of the Director of Nursing, and the Hospice Physician in the provision of skilled nursing care & services, assessment, planning, and care in order to maximize the comfort and the health of patients and families. The RN is responsible for providing nursing care and services, assessing and identifying patient/family needs, and providing skilled care and supportive services in accordance with the patient’s physician’s orders and plan of care and Hummingbird Hospice policies and procedures.​

QUALIFICATIONS:

1. Graduate of an accredited school of nursing.

2. Valid license as a Registered Nurse in the State of Texas

3. Minimum of 2 years Home Health and/or hospice experience required

4. Current Basic Life Support Certification

5. Valid state Driver’s License

6. Able to cope with emotional stress and be tolerant of individual lifestyles

7. Sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death.

8. Basic computer skills and Kinnser experience required

JOB REQUIREMENTS

1. Assess home care needs, be aware of the physical, emotional, and spiritual aspects, and gather data on social, economic, and cultural factors which may influence health, well-being, and quality of life.

2. PRN and on-call availability

3. Constant correspondence with patients, caregivers, and families.

4. Provide direct care to patients per the Interdisciplinary Plan of Care.

5. Evaluate and perform admissions, ongoing assessments, recertifications, discharges, and communication.

6. Complete new orders or refills for medication, DME, imaging, labs, transportation, therapies, infusions, podiatry, nutrition, phlebotomy, or others as needed.

7. Submit documentation notes timely and accurately

8. Assist with training new hires and interns as needed

9. Participate in the agency’s on-call rotation as prescribed by the needs of the agency to provide nursing service to clients when required outside office hours.

10. Be available, when possible, to meet a patient/family's need for continuous care in time of crisis.

11. Coordinate community resources and other agency disciplines participating in patient care.

12. Maintain effective communication with other hospice personnel involved with patient care.

13. Availability to perform patient intakes and information visits as needed including an explanation of the hospice benefit/Medicare, complete physical assessment, completion of all pertinent paperwork, and communication of new patient status to the IDG team.

14. Performs other duties as assigned consistent with skills and training and the mission and goals of the agency.

Pay: $65.00 - $145.00 per hour
