POSITION SUMMARY:

The Vista Program Assistant, in conjunction with other team members and the Program Director, is responsible for the special service and care required for each Program member. Assist with member care and program/activity needs while providing a positive physical, nutritional and social environment for members, ensuring they feel safe and purposeful.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. 

With the Program Director create, organize, and facilitate therapeutic and purposeful, engaging activities allowing for relaxation, exercise, and socialization 

Motivate and encourage members to become involved in such activities 

Initiate and lead small group, large group and one to one activities with members 

Ensure that supplies for all activities are readily available and activities are set up for members 

Provide cueing and reminding with ambulation, meals and toileting 

Provide medication reminders for members as needed 

Assist with "household chores" and encourage participation of interested members 

Ensure that all rules, regulations and safety practices are properly executed 

Perform work in accordance with the facility standards and policies regarding risk management, safety, infection control, etc. 

Assist with facilitation of Vista Dining Program. Serve appropriate snacks and beverages 

Use proper sanitary procedures to wipe tables and counters and sweep dining room floor after meals and snacks 

Participate in quality improvement initiatives 

Participate in staff meetings 

Provide emotional support to members and family caregivers. 

Provide a positive and supportive environment to members. 

Document in Communication Book per Program policy 

Ensure members' safety; follow health and safety policies; anticipate hazards, and take appropriate measures to respond. 

Respond to emergency situations calmly and effectively 

Report any change in member’s health or condition to Program Director 

Report environmental hazards to Program Director 

Demonstrate respect and compassion in every interaction with members, peers and family caregivers 

Conduct oneself with the highest degree of honesty and integrity in every interaction 

Demonstrate a passion for caring as evidenced by interactions with co-workers, members , families, and visitors 

Maintain the confidentiality of all member and family information 

Attend and participate in mandatory training and in-services

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required. Educational Requirements and Experience: 

Must have current certification as a nursing assistant or home health aide 

High School education preferred 

Possesses current First Aid, and Heimlich certification, if required by state regulations


Knowledge, Skills and Abilities: 

Language Ability: Ability to read, write, speak and understand the English language. Knowledge of the structure and content of the English language including the meaning of words, rules of composition, and grammar. Ability to effectively communicate with peers, supervisor, residents, clients, and guests. 

Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measurement, using whole numbers, common fractions and decimals. 

Cognitive Demands: Ability to apply common sense understanding to carry out instructions provided in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Able to swiftly change direction as necessary.


Reliability including reporting to work as scheduled and on time, and completing assignments according to facility policy.
