Harbor Health Home Care has an opening for an RN Case Manager servicing St. Clair and Macomb counties. The position is full-time pay-per-visit with benefits. Interested applicants need one year of med/surg and prior home care experience is preferred.

Candidate must be highly motivated and organized. We are seeking a professional who provides skilled nursing services to patients in their residences, on an intermittent basis. RN evaluates, plans and implements care to rehabilitate the patient, seeking to promote optimal level of outcome. Confers with physicians and other members of the health care team to formulate realistic goals and evaluate nursing program.

This position is an exciting opportunity to work with individuals to help improve their independence by designing nursing regimen; instructing/in-servicing individuals, families, and caregivers; recommending that assists, or home health follow-up programs.

*Knowledge of nursing principles, theory, methods and evidence based practice.

*Knowledge of effective practice, clinical decision making, outcomes based intervention and

their proper application in the educational environment.

*Knowledge of administrative management practices.

*Ability to plan, implement, administer and coordinate a skilled nursing program, from

eligibility determination to exit.

*Ability to select and administer appropriate assessment tools and interpret results of

assessment.

*Skill in selecting and providing a wide range of intervention strategies and monitoring their

effectiveness.

/Ability to plan, coordinate and provide training and continuing education both formally and informally to patients, their families, community agencies, staff and other medical professionals.

*Skill in effective oral and written communication.

*Knowledge of current legal mandates, including: Federal and State law, compliance issues,

confidentiality requirements.

Educational Requirements:

*Graduation from an accredited program of Nursing

*Certification to practice in Michigan as evidenced by current license as a Registered Nurse(RN) by the Michigan Board of NURSING.Competitive wages based on experience.
