Job Description
The role of the RN Case Manager is to manage an assigned caseload of patients to ensure optimal clinical, financial and quality outcomes during acute hospitalization. The RN Case Manager utilizes clinical criteria, professional nursing skills, and experience to assess admission and continued stay appropriateness and monitor the care plan to ensure timely delivery of care and discharge planning. The RN Case Manager will proactively identify patients with post-discharge needs and collaborate with the patient/family, medical team and Payer to coordinate and facilitate the discharge plan.

Education
- Associate or Bachelor's degree in Nursing from an accredited college or university. [Required]
- Current Indiana Registered Nurse licensure. [Required]
