POSITION SUMMARY:
The purpose of this position is to provide skilled nursing care to patients requiring professional nursing service in accordance with physician’s orders and in compliance with state and federal regulations.
ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including but not limited to:
- Obtains a medical history, related to the patient's present condition
- Evaluates the patient's physical, mental and emotional status, environmental factors that support/impede home care and the presence of a permanent caregiver in the home to assist with daily care
- Develops and implements the nursing care plan
- Supervises and evaluates the care given by the home health aide as
- Participates in Interdisciplinary Team Meeting (IDT) and collaborates with members of the team to promote quality patient care
- Participates in rotating on call
- Willing to travel to other office locations to assist with staffing if needed
- Makes recommendations and demonstrates follow-up on assessment findings appropriately
- Maintains proficiency with laptop computer
Required Skills:
- Able to interpret and implement regulatory guidelines
- Ability to work in stressful environments/situations
- Ability to make decisions concerning appropriate patient care
- Ability to respond appropriately to life threatening situations and critical thinking skills
Required Work Experience:
- Minimum of one (1) year as a Registered Nurse
- Homecare/Hospice experience preferred
Licensing/Certification/Education Requirements:
- Graduate of an accredited school of nursing with a Registered Nursing (RN) degree
- Current State Registered Nurse (RN) License
- Current State of Tennessee Registered Nursing (RN) License
- Current CPR Certification
Other Requirements:
- Position may require unscheduled overtime, week-end work
- Must be willing and able to work flexible hours
- Travel required
- Must have reliable transportation
- Current State Driver’s License
- Current Auto Liability Insurance according to company policy

Physical Requirements:
- Work requires the physical demands of standing, bending, lifting, stooping or performing other work requiring heavy physical exertion (over 50 pounds) on a consistent basis (up to 15% of the time) and frequent walking or moderate physical exertion (30 to 50 pounds) on an intermittent basis
- Work is performed under basically normal conditions as of a standard hospice environment
- Work may involve possible exposure to biohazardous waste, chemical substances and unknown diseases
- Care is provided in a variety of home environments with variable temperatures
- Possible exposure to animals in the home, driving traffic, road hazards and possible inclement weather
