Who We Need
This position is located at our West Valley Integrated Health Clinic in Peoria AZ.
As a member of the Medical Assertive Community Treatment (MACT) Team, the MACT Rehabilitation Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. Works as a team member of a clinical team to assess consumer needs and wants, participates in the development of individual service plans, acts as a consumer advocate and liaisons with families and community agencies. Serves as the team’s specialist for rehabilitation services by collaborating with other team members in the assessing for and delivery of rehabilitation skills training as specified in the individual consumer’s service plan. Provides on-call crisis intervention services, as well as weekend and holiday coverage on an established schedule. Flexible work schedule is required.
MINIMUM QUALIFICATIONS:
- High School diploma/GED and four plus (4+) years direct care experience, or;
- Any combination of direct Behavioral Health experience/education totaling five (5) years of experience is acceptable. Example: Bachelor’s degree with one (1) year of experience.
- One (1) year education and one (1) year professional experience in, rehabilitation treatment and recovery.
- Must complete training and demonstrate competence in Integrated Care skills.
- Must have demonstrated experience in Microsoft Office applications.
- Satisfactory work records as determined by the People 1st
- Successfully completes a pre-employment drug screening.
- Complete all on-boarding paperwork, in a timely manner and be cleared to attend New Employee Orientation (NEO), as scheduled.
- Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
- Ability to meet Copa’s driver authorized driving requirements, set forth by Copa Health’s insurance company. Including AZ Driver’s license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable.
- Bi-Lingual, preferred.
JOB SUMMARY (Including, but not limited to)
- Works with consumer’s Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
- Participates in the service plan goal development for each consumer as it related to their rehabilitation goals and the interventions they will need.
- Provides and oversees the provision of any direct skills training(s) indicated as needed by the individual MACT consumers.
- Works in close collaboration with the Independent Living Skills Specialist, Housing Specialist, Employment Specialist, and Substance Abuse Specialist to ensure coordination of services.
- Assumes “primary contact” responsibility for a percentage of team consumers to: assure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individuals; keep weekly client schedule for those individuals updates, and act as first contact for those consumers and their families.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department, consistent with Copa Health productivity levels
