Basic Purpose of This Position

The purpose of this position is to provide delivery of care as described on each individual resident’s care plan and as directed per nurse aide training standards, maintaining the residents room in a tidy, orderly condition with respect to the individual residents rights and preferences, serving resident meals and nourishments including assistance with setting up of meals, dining room supervision and serving meals, reminding residents of activities the facility, completing and documenting all tasks as directed on the individual resident’s care plan such as direct care delivery, temperatures and weights, maintaining the required annual (12 )continuing education hours, reporting unusual occurrences for resident to the unit charge nurse, reporting incidents and accidents, completing resident showers, if independent, supervising shower time, documenting all changes to care plan and informing supervisor and PC Director of changes and assisting with other tasks as assigned.

Position Specific Duties

· Delivers care as described on each resident’s care plan and as directed per nurse aide training standards. Discusses change in acuity with Supervisor and PC Director to change plan when needed

· Maintains the residents’ rooms in a tidy, orderly condition with respect to residents’ individual rights and preferences

· Serves residents meals in the dining room and nourishments on the unit

· Transports residents to the dining room, activities, and/or beauty salon as needed per their request and/or schedule

· Supply fresh ice, water and other refreshments as assigned

· Completes all tasks as directed on the unit’s assignment sheet for the assigned shift

· Reports any unusual occurrences (incidents), signs, symptoms, complaints or observations of the resident to the unit charge nurse.

· Documents as required per facility policy including, but not limited to:

o Resident’s care delivery on the care plans

o Resident’s Care Plan Intervention Sheets

o Resident’s Shower Days and Skin Assessment

o Resident’s temperature (as appropriate)

o Resident’s weight (as assigned)

· Reports any allegation of abuse, neglect or misappropriation of resident property per facility policy

· Follows the policies and procedures of the facility

· Assists with the orientation of new employees as directed

· Attends staff meetings as directed

· Completes Laundry according to scheduled days

· Pulls trash at end of shift and as needed

· Performs other tasks as assigned by the unit charge nurse.

Education, Specialized and/or Technical Knowledge Requirements

· Possesses a current, active certification as a Nurse Assistant in the state of Kentucky (optional) or has completed training through IVY KNOLL

· Demonstrates knowledge of and ability to apply principles of nursing care.

· Demonstrates the ability to work effectively with all levels of personnel

· Demonstrates the ability to utilize appropriate judgement in requisitioning assistance from licensed personnel.

Job Type: Part-time

Pay: $12.00 - $13.50 per hour

Experience:

• Caregiving: 1 year (Preferred)

License/Certification:
