RN Clinical Manager Job at Seva Hospice

We are currently looking to add a Clinical Manager with Hospice experience to join our hospice team. Our service area includes Stanislaus, San Joaquin, and Merced Counties.
The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately, and that care and services are delivered appropriately and for the supervision of clinical personnel.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Receives hospice referrals, assigns appropriate personnel and conferences with the hospice medical director to determine eligibility.
Reviews cases on an ongoing basis to assure delivery of high-quality, evidence-based care.
Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course and assists clinicians with developing the plan of care and setting goals for quality outcomes.
Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees and ongoing staff education and in-services.
Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel.
Takes a lead role in quality assessment performance improvement teams and activities.
Performs other duties and activities as delegated by the Director of Patient Care Services.
POSITION QUALIFICATIONS
Registered nurse with current licensure to practice professional nursing in California.
Graduate of accredited school of nursing with a BSN degree is preferred.
Minimum 2 years previous experience in a hospice setting
2 years management or supervisory experience.
Proven ability to work within an interdisciplinary setting.
Complies and maintains current CPR certification.
Demonstrates excellent observation, verbal and written communication skills.
