The Senior Intake Nurse will be responsible for supervising and managing day to day intake operations, to include scheduling and team support. Will be responsible for initiating team leadership and support to facilitate the daily Intake functions, tasks, and standard operating practices of the Intake Department. Will also function as an HHS adult intake nurse with responsibility of initiating and processing patient referrals to assure that the patient needs are met, and agency policies are followed. Establishes and maintains ongoing relationships with referral sources, payers, and case managers. Will ensure smooth operations to prevent back-ups in the EPIC Intake workflow process. The senior intake nurse will set a tone and environment for positive employee satisfaction and engagement. This senior position will be responsible for fostering an environment of workflow efficiencies and accuracy through education and mentorship. Will work with Clinical Operations manager to identify areas of improvement, act promptly to establish resolutions that will maintain or increase workflow efficiencies. Will be designated back up for Clinical Operations Manager.

Essential Job Functions

  • Responsible for processing incoming patient referrals for various services accurately and concisely. Screen and exchange information with caller to determine if patient lives in an area service by JHHCG. Determines if patient meets agency admission criteria and /or federal and state guidelines. Probes to identify expressed and implied needs to determine JHHCG's ability to meet caller’s needs.
  • Identifies needs for other disciplines/services. Assist in establishing and maintaining standards set forth to develop initial plans of care to meet the patients' needs and facilitate transitions of care through the entire EPIC Intake workflow system. Facilitate safe transitions in care by adhering to the SOPs of the Central Intake Division: specifics on page 3 of 5. Participates in initiatives to promote smooth transitions and the highest level of patient care.
  • Responsible for communicating to the referral source avoiding any barriers to providing the necessary care. Will assist in resolutions of problems, provide ongoing education to offsite HCC to promote unity of practices that meet agency standards and policies, assist to ensure accuracy of Home Care Referrals.
  • Responsible for physician orders on initial referral. Identify follow up vs. referring physician, responsible for physician orders by phone, fax, or addendum in EPIC system and forward to the appropriate team. Responsible for entering the referral information and physician orders into EPIC Electronic Medical Record Measures. Completes clear and concise notes and physician orders in EPIC, and referrals completed on a timely based start of care.
  • Other duties as assigned, will participate in general orientation of all new HHS employees as needed, precepts all NEW intake nurses, serves as Expert in intake process and work flows, acts as a resource for the Intake team and facilitates continuity of SOPs within EPIC functions, volunteers as needed, provides education and guidance for all intake nurses as team lead, ensures that intake flows operate efficiently, safely and at a peak service standards, and participates in new hire interview.

Education

  • Registered Nurse. AA or Diploma required; BSN preferred.

Required Licensure, Certification

  • Maryland Licensure as a Registered Nurse.

Work Experience

  • A minimum of three (3) years clinical experience.
  • One year of work experience must be in a position of responsibility (Administration/Supervision/Preceptor), preferable in a case management, community health or home health setting, HME, homecare for Medicare certified agency, and infusion experience preferred.

  • Office and telecommunications equipment, EPIC electronic medical record system.
