Responsibilities:
Under the general supervision of the Program Director, the Social Worker provides social services, in accordance with Title 22 mandates, to assist participants and their families to solve medical and psychological problems that arise as a result of the participant’s disability.
Essential Duties:
1. Respond to inquiry calls from prospective consumers, professionals and community referral resources;
2. Conduct initial screening to determine appropriateness of referral;
3. Conduct program tours for prospective participants and caregivers, professional referral sources and community members;
4. Conduct initial in-home assessment with prospective participants and continuing assessments of participants through the formation of an individual plan of care in conjunction with the Inter-Disciplinary Team;
5. Coordinates admission and assessment process with admission clerk, program director and Inter-Disciplinary Team;
6. Provide counseling to participants and families as needed;
7. Provide scheduled in service trainings to program staff based on subjects in conjunction with or identified by the Program Director (PD);
8. Work with program participants when requested by the Activity Coordinator, regarding issues such as when a participant is exhibiting challenging behaviors or having difficulty being redirected;
9. Coordinate and schedule time to work with participants with the Activity Coordinator in advance, ensuring smooth coordination resulting in a mutually desired outcome on Social Work issues;
10. Coordinate the following required meetings: Rehabilitation Planning upon admission, Rehabilitation Review monthly, Utilization Review committee quarterly;
11. Document progress of each participant through an Individual Plan of Care (IPC), quarterlies, reassessments and progress notes in the computer and in the participant’s chart, meeting all deadlines;
12. Provide individual and group counseling in consultation with the contract psychologist;
13. Coordinate referrals to other community resources and provides a liaison to these resources;
14. Initiate and coordinate all required documentation, i.e., doctor’s referral and information for admission, Treatment Authorization Reports, Discharge Plans and follow-up, meeting all deadlines;
15. Provide referral for persons not appropriate for Adult Day Health or persons discharged from ADHC;
16. Lead in-house support group for participants and monthly support group for caregivers as available;
17. Establish and maintain relationships with appropriate professional and community based referral resources through a systematic outreach effort including in-person presentations and meetings, staffing health fairs and community events, mailings, telephone contacts, email and other strategies as needed;
18. Provide information and education to medical professionals and community based resources regarding Adult Day Health Services.
Qualifications:
1. A Master’s degree in social work from an accredited college or university and one year’s experience as a Social Worker in a health related field, with demonstrated knowledge of case management OR a Bachelor’s degree in social work & 2 years experience as a Social Worker in a health related field, with demonstrated knowledge of case management;
2. Possess knowledge of community resources for the disabled and elderly;
3. Must have clean DMV record, reliable personal transportation, and auto insurance;
4. Must be able to communicate clearly in English both verbally and in writing;
5. Ability to work independently as well as in a team environment.
Hours: 8:30AM – 3:30PM
