Applicant must be MERIT certified through the Department of Human Services.

  • Conduct Family Assessments within designated timeframes
  • Determine if on-going CP services are needed
  • Provide on-going case management, case planning, service coordination and monitoring as needed
  • Conduct home visits with child and parents at least monthly
  • Complete all necessary documents (SDM tools, child well-being tools, mental health screening tools, case notes, etc.) as required by SSIS, Minnesota statute, and federal Children and Family Service Reviewed within required timeframes
  • Monitor and move forward with cases that may not be progressing well and may need more intensive intervention (switch to traditional investigation; file CHIPS, etc.)
  • Handle and respond to crisis situations as needed
  • Develop Service Plan (CP In-Home Plan; OHP Plan, CW General SS Plan, etc.) in conjunction with parents, children and other appropriate stakeholders within required timeframes
  • Provide on-going case management; coordinate, arrange and monitor service provision; track and evaluate progress toward goals
  • Conduct face-to-face visits with children and parents at least monthly
  • Complete all necessary documentation (SDM tools, child well-being tools, mental health screening tools, placement entry, VPA form, case notes, etc.) as required by SSIS, Minnesota statute, and federal Child and Family Service Reviews within required timeframes
  • Report to the court as needed on CHIPS cases; coordinate closely with assigned GAL
  • Follow all current state permanency timelines and IV-E procedures for children in placement
  • Handle and respond to crisis situations as needed
  • Interview child sexual abuse or egregious harm victims in concordance with forensic interviewing guidelines (ex. Cornerhouse methodology)
  • Coordinate with County Attorney and Law Enforcement to ensure a coordinated investigative response with little or no duplication of activities
  • Complete all steps required for Traditional Investigation
  • Coordinate with assigned case manager to successfully transition case to on-going case management if needed
  • Assess child and student situation to determine most effective course of intervention for each individual; coordinate and provide referrals as needed (ex: chemical dependency evaluation, mental health evaluation, etc.)
  • Develop service plan in conjunction with parent, truant teenager, and other appropriate stakeholders within required timeframes
  • Coordinate in an on-going capacity with stakeholders, collaterals and referral sources as needed (schools)
  • Provide on-going case management; coordinate, arrange and monitor service provision; track and evaluate progress toward goals; adapt plan and respond to client change in need in a flexible manner
  • Conduct face-to-face visits with children at least monthly
  • Complete all necessary documentation (SDM tools, child well-being tools, mental health screening tools, placement entry, VPA form, case notes, etc.) as required by SSIS, Minnesota statute, and federal Children and Family Service Reviews within required timeframes.
  • Report to the court as needed in a timely manner
  • Follow all current state permanency timelines and IV-E procedures if a truant child does end up in placement
  • Hand and respond to crisis situations as needed
