Agency vehicles may be available; however, mileage is reimbursed for some activities requiring use of personal vehicles. The selected applicant must be able to attend approximately five working days of staff development orientation in Lytle, TX.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

Under the supervision of the Community Support Supervisor, this position works as a Provider Coordinator to a caseload of individuals receiving waiver services in Zavala County. Duties include monitoring to ensure that the individual is receiving services as outlined in their Person Directed Plan and Individual Plan of Care. Develops Implementation Plans and reviews all services with the individual monthly. This position develops and maintains a system of tracking to ensure that the above-mentioned activities occur in a timely manner. This position also provides care, training, transportation, and employment services. Staff develops strategies to coincide with the individuals Person-Directed Plan and completes appropriate documentation. Completes Employment Assessments to help guide employment services for the consumer. Duties include working with the consumer to ensure opportunity to succeed in potential employment. The provider coordinator technician must train in self-help skills, pre-vocational skills, and special therapy and behavioral techniques, as directed by professional staff. Must be willing to work flexible hours, which include evenings and weekends. Must have a Texas Driver’s license, acceptable driving record and dependable transportation. Establishes positive relationships with individuals, families, community members, business community, and staff. Will provide services within a wide range of activities in accordance with the requirements of the program. Staff will appropriately maximize revenues by ensuring that all service provision activities are documented correctly and submitted within the agency-specified time. Will clearly understand the billing guidelines for the waiver programs and maximize billing through monitoring documentation. Staff will conduct agency business in an honest, fair, professional, and integrity-based manner.

MINIMUM JOB QUALIFICATIONS:

· Graduation from an accredited high school or GED with a minimum of two years’ experience in a field related to the duties of the position.

· OR: An Associate’s degree in social, behavioral, or human services or related field.

· Must maintain a clear driving record (license, defensive, liability insurance) and personal vehicle to transport individuals.

PREFERRED JOB QUALIFICATIONS:

· Lives in the service area.

· Speak, read, and write in both English and Spanish.

KNOWLEDGE, SKILLS, AND ABILITIES:

· Knowledge of appropriate intervention techniques for individuals with intellectual and developmental disabilities of all ages and ability to use these effectively.

· Ability to coordinate and plan programs and provide employment services for individuals.

· Ability to work as a team.

· Must possess good computer skills.

· Skillful in developing, organizing, and utilizing community services.

· Must possess excellent communication and writing skills.

· Ability to resolve conflict in area of responsibility.

· Knowledge and understanding of Medicaid Waiver programs.

· Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.

· Strong communication skills.
