HOURS: Part-time hours in 2.25 Days - Friday through Sunday - Every other weekend
This position is part-time, is every other weekend, and starts on Friday at 7:00 pm and ends on Monday at 12:30 am (Can start earlier or later based upon the individual needs at a Community Home).
This is a 24-hour shift position with on-site sleep hours of 11:00 pm-7:00 am, Friday into Saturday, and Saturday into Sunday.


FUNCTION:

The Weekend Coordinator, of Community Living, is responsible for supporting people with intellectual and developmental disabilities. Together with the person and their family, assist in charting a course that supports each Person in realizing their goals and dreams and achieving an everyday life that is important to them. This position coordinates activities, outings, and supports for each weekend.

RESPONSIBILITIES:

  • Contribute to the development of Person-Centered plans. Understands and implements each Person’s Person-Centered Plan and current ISP that reflects the goals and outcomes chosen by the Person.
  • Respect each Person’s right to make choices and exercise self-determination, resulting in life experiences and personal growth.
  • Support each person’s health and safety, while also supporting their right to risk.
  • Provide opportunities for each person to explore their interests in their community and build relationships.
  • Understand the importance of family in each Person’s life. Build a positive relationship with family members.
  • Develop positive working relationships with team members.
  • Support each Person’s daily routine to include personal care, appointments, leisure activities, and transportation.
  • Coordinate activities, outings, and support for each weekend. Plans with People in advance and communicate that plan with the Community Living Supervisor for approval.
  • Complete the required documentation.
  • Complete the Medication Administration training and annual practicums successfully. Complete Medication Administration documentation as trained.
  • Participate in training and meetings as required.
  • Adhere to all policies, procedures, and regulations.
  • Perform other related duties as assigned. The equivalent of an Associates Degree in Human Services/Social Science. Able to complete one-person transfers. Effective communication, interpersonal and planning skills are required. Satisfactory Act 33 and 34 clearances and FBI as required. Valid license to drive in the State of Pennsylvania and properly insured vehicle.
